Usually. On clay soils it's typically a grading problem, sometimes combined with downspouts discharging in the wrong place. The fix ranges from re-grading to a French drain or dry well — we diagnose it on site rather than prescribing before we've seen it.

Usually, yes. Ontario's Building Code generally requires a permit once a deck sits more than about 600 mm above grade, and the City of Mississauga applies its own requirements on top of that. We confirm what's needed with the local building department before the build, and our footings go below the frost line either way.

Yes. Ontario law requires a locate before anyone digs, so we submit an Ontario One Call request and wait for the utility locates to come back before excavation starts. It costs you nothing and it's not a step we skip.
